The Lancet published TRANSCEND-T2D-1 on 13 June: a 40-week, double-blind, placebo-controlled phase 3 trial of retatrutide, a GIP, GLP-1 and glucagon receptor triple agonist, given as monotherapy to adults whose type 2 diabetes was inadequately controlled by diet and exercise alone. 537 participants were randomised across 48 sites in the United States, Mexico and India, to one of three dose levels or placebo.
The primary endpoint was change in HbA1c at week 40. All three retatrutide groups separated from placebo, with estimated treatment differences of roughly 0.9 to 1.1 percentage points. Mean bodyweight change ranged from about —11.5% to —15.3% against —2.6% on placebo. The most frequent adverse events were gastrointestinal and generally mild to moderate, and no severe hypoglycaemia was reported. The trial was funded by Eli Lilly and Company.
