CagriSema beats semaglutide on HbA1c in REIMAGINE 2

The amylin and GLP-1 fixed combination was superior to semaglutide alone over sixty-eight weeks in 2,713 participants across thirty countries.

Lancet Diabetes and Endocrinology has published REIMAGINE 2, a 68-week phase 3 study of cagrilintide-semaglutide against semaglutide alone, cagrilintide alone, or placebo, in people with type 2 diabetes and overweight or obesity. 2,713 participants were randomised across 30 countries, all of them already on metformin with or without an SGLT2 inhibitor.

On the primary endpoint, change in HbA1c at week 68, the combination at 2.4 mg of each component was superior to semaglutide 2.4 mg. The authors report a safety profile consistent with the GLP-1 receptor agonist class and with earlier cagrilintide data. REIMAGINE 1 and REIMAGINE 3 were published alongside it. The programme was funded by Novo Nordisk.

Catalogue note

Cagrilintide is MET-04 and the fixed co-formulation with semaglutide is MET-05. Both ship only as sealed ten-vial kits to verified research organisations, released against a single specification, labelled Research Use Only and not for human consumption. As noted in June, cagrilintide also cannot lawfully be used in compounding in the United States.
